This Sporting Registration contains a waiver and release of liability and indemnity agreement attached to which you will be bound, and under which you will be waiving important legal rights.

The parent(s) or legal guardian(s) will assume any and all risks of injury associated with or in any manner related to the participation in the assigned sport the child participates in. This assumption will be at a game (either home or away), practice, or while traveling to either practice or a game.

The parent(s) or legal guardian(s) will assume any and all risks of injury associated with the usage of any equipment provided by Calvary Athletic Association.

The waiver applies regardless of whether the child is using equipment provided by Calvary Athletic Association at the time of the injury or whether the injury occurs as a result of your presence upon the premises regardless of the purpose or activity at the time of the injury causing incident.

The parent(s) or legal guardian(s) agree not to sue or otherwise attempt to hold the club liable for any injuries regardless of the cause.

The parent(s) or legal guardian(s) agree to indemnify and hold the club harmless against any and all claims arising out the child’s presence upon the club’s premises, including the use of the premises by any family member or guest of the child regardless of the cause.

This waiver and release agreement applies to all injuries including, but not limited to slipping in or around the facility and injuries by malfunctioned equipment.

This waiver and release agreement is intended to be as broad as is allowed under the applicable law and applies to any and all claims for damages, regardless of whether they are allegedly caused by the negligence of Calvary Athletic Association or any of its’ volunteers.
